Join us for a call with CMSA's National President, Mary McLaughlin-Davis and Rebecca Perez Director of Product Development present: "Population Health and the Role of the Integrated Case Manager" Population Health is the latest initiative to improve the health of all Americans. This model moves from a traditional model of reactive care to one that is proactive, addressing health needs earlier and solving for the barriers that prevent improved health. As healthcare providers, systems, and payers move to this model, the role of case management will be integral to its success, but case managers must be better prepared to work with complex populations that will be included as subpopulations in the Population Health model.
